Hello,
Is it possible for a male to female Transgender male to have the change at the age of 60. There's a lot of internet information about "Early Transitions", but the only discussions I can find about transition over 40 seem to range somewhere from "be realistic" to "don't do it", even though there seem to be a number of references to successful over 40 transitions.

You are not alone or late, the early transitioners are just more prone to posting websites. You are right on time! Actually, most transgender people don't start coming out until almost precisely 40, for a variety of reasons. Many people have successfully transitioned well into their sixties and seventies.

Transition is a real option, but it is not easy, nor is it cheap. It is also not the only option. There are many places in between, many people transition in stages. You need to find where YOU are comfortable. Some people just let their hair down, others won't be satisfied until they have rewritten their DNA. Take the time to find out what you want, it is up to you.

I am married and my wife is totally behind me and I am thinking of breast augmentation. What would I be looking at as far as resistance from doctors or whatever? I'm confused.

I assume from your letter that you do not plan on transitioning. As teenage girls know, developing breasts are hard to conceal. Be sure you want them ALL the time.

Breast augmentation can be achieved with implants and/or hormones. There are numerous surgeons performing implants, so you can shop around. Finding a knowledgeable endocrinolist for hormones may be a bit more difficult.

For better or worse, many medical professionals are reluctant treat transgender people without a letter from a therapist. This may be harder to obtain if you don't plan on transitioning. Fear of lawsuits (not unfounded) and lack of knowledge inhibits them.

Last but not least, outright prejudice is not uncommon. Many medical treatments transgender people request are routinely available to women, but some doctors refuse to work with transgender patients, claiming they don't know how to deal with "us".

Dear Nick,
We are not aware of any specific financing options for our surgeries. Ultimately, there is no collateral for a loan. A few banks are starting to offer loans for elective surgery, but this is a brand new industry.

For those lucky enough to own their own home, an "improvement" loan may offer the best terms and tax advantages. Credit cards are probably the most readily available method for many people, although the terms are generally quite severe, especially if you make a late payment. If you can't get a credit card, you probably would have a hard time securing other types of financing in any case.

You should at least start putting money aside each month, as if you were making payments on a loan. If you don't find financing, eventually you will have the money anyway. Medical savings accounts, for those eligible, may also offer tax advantages.
